| 摘 要: 目的:通过观察益气活血方对机械通气肺损伤大鼠血清、肺泡灌洗液及肺组织中Ghrelin、AngII、肾素、IL-6、TNF-α表达的变化还有其对肺组织的影响，试发现其对肺组织的保护作用及机制。方法:将40只SD大鼠随机分成空白组，模型组、中药组和阳性药物干预组，乙醚麻醉后用动物人工呼吸机通气的方式制备机械通气肺损伤大鼠模型，其中阳性药物干预组造模前腹腔给药Ghrelin，中药组在通气后给予益气活血方灌胃2周，观察肺组织结构及炎症细胞浸润程度，ELISA法检测Ghrelin、AngII、肾素、IL-6、TNF-α在各组大鼠血清和肺泡灌洗液中的表达，RT-PCR法检测肺组织 IL-6、TNF-αmＲNA表达。结果：与空白组比较，造模后，机械通气肺损伤模型组大鼠血清及肺组织中AngII、肾素、IL-6、TNF-α均有不同程度水平升高，Ghrelin表达水平降低（P＜0.05或P＜0.01）。给药2周后，和模型组相比，中药组AngII、肾素、IL-6、TNF-α表达水平略微下降，Ghrelin表达上调 (P＞0.05)；HE染色病理提示两给药组大鼠肺组织形态结构明显改善。结论：益气活血方对VLLI大鼠肺组织有保护作用，可能与下调VLLI大鼠肺组织中AngII、肾素、IL-6、TNF-α，上调Ghrelin表达水平，减轻了炎症反应对肺组织的损伤有关。 |

| Aim: To observe the effect of Yiqi Huoxue Decoction on ghrelin, AngII, renin, IL-6 and TNF-α in serum, bronchoalveolar lavage fluid and lung tissue of rats with mechanical ventilation induced lung injury (VILI) ，In order to elucidate the possible mechanism of lung protection. Methods: 40 SD rats were randomly divided into blank group, model group,traditional Chinese medicine group and positive drug intervention group, The positive drug group was given ghrelin intraperitoneally before modeling, and the traditional Chinese medicine group was given Yiqi Huoxue Decoction by gavage for 2 weeks after modeling. The lung tissue structure and the degree of inflammatory cell infiltration were observed. The levels of ghrelin, AngII, renin, IL-6 and TNF were detected by ELISA- α The expression of IL-6 and TNF in serum and BALF were detected by RT-PCR- α MRNA expression..Results: Compared with the control group, the levels of AngII, renin, IL-6 and TNF-αin the serum and lung tissue of the model group were significantly higher than those in the control group，The expression of ghrelin was decreased (P＜0.05 or P ＜0.01). After 2 weeks, compared with the model group, the levels of AngII, renin, IL-6 and TNF-αin the traditional Chinese medicine group were significantly higher The expression of ghrelin was up-regulated (P＞0.05); and hematoxylin-eosin staining showed that the morphology and structure of lung in two groups were significantly improved.Conclusion: Yiqi Huoxue decoction has protective effect on lung tissue of VILI rats, which may be related to the down-regulation of AngII, renin, IL-6 and TNF-α in lung tissue of VILI rats，Up regulation of ghrelin expression can alleviate the lung injury caused by inflammatory reaction. |
