| 目的 观察祛瘀化痰二仙汤联合非布司他对高尿酸血症患者阴茎血流动力学及勃起功能障碍的影响。方法 选取高尿酸血症伴勃起功能障碍患者90例，采用信封法随机分为联合组（n=45）和对照组（n=45）。对照组口服非布司他，联合组在对照组基础上加服祛瘀化痰二仙汤，连续治疗12周。分析比较两组治疗的有效性和安全性；采用彩色多普勒超声检测阴茎血流信号，对比分析两组治疗前后阴茎背动脉收缩期最大流速（PSV）、舒张末期流速（EDV） 、血管阻力指数（RI）、阴茎血流指数（PFI）；检测比较两组治疗前后血浆人内皮素-1（ET-1）、血浆一氧化氮（NO）等血管内皮功能；运用国际阴茎勃起功能指数评分（IIEF-5）判断两组治疗前后的勃起变化。结果 治疗前两组IIEF-5评分和阴茎血流动力学以及血管内皮功能指标水平比较差异无统计学意义。与治疗前相比，两组治疗后RI、PFI、血浆ET-1下降，而PSV、EDV、血浆NO和IIEF-5评分提高（P＜0.05）。与对照组比较，联合组治疗有效率提高，治疗后的RI、PFI、血浆ET-1下降，PSV、EDV、血浆NO和IIEF-5评分提高，并优于对照组（P＜0.05）。联合组和对照组均无明显不良反应或并发症发生。结论 祛瘀化痰二仙汤联合非布司他可有效改善高尿酸血症患者血管内皮功能和阴茎动脉血流，从而勃起功能障碍，未增加不良反应。 |

| Objective To study the effects of Quyu Huatan Erxian Decoction combined with febustat on penile hemodynamics and erectile dysfunction in patients with hyperuricemia.Methods Ninety patients with hyperuricemia and erectile dysfunction were selected and randomly divided into a combined group (n = 45) and a control group (n = 45) by the envelope method. The control group was orally administered febuxostat, and the combination group was treated with Quyu Huatan Erxian Decoction for 12 weeks. Analyze and compare the effectiveness and safety of the two groups of treatments; use color Doppler ultrasound to detect penile blood flow signals, and compare the two groups before and after treatment of the maximum velocity of vasoconstriction (PSV), end diastolic velocity (EDV), and vascular resistance Index (RI), penile blood flow index (PFI); detection and comparison of plasma endothelin-1 (ET-1), plasma nitric oxide (NO) and other vascular endothelial functions before and after treatment in two groups; using the international penile erectile function index score (IIEF-5) Judge the changes of erection before and after treatment in the two groups.Results There was no statistically significant difference in the IIEF-5 score, penile hemodynamics and vascular endothelial function indexes between the two groups before treatment. Compared with before treatment, RI, PFI, and plasma ET-1 decreased after treatment, while PSV, EDV, plasma NO, and IIEF-5 scores increased (P <0.05). Compared with the control group, the combination group"s treatment efficiency increased, after treatment, RI, PFI, plasma ET-1 decreased, PSV, EDV, plasma NO and IIEF-5 scores increased, and was better than the control group (P <0.05). There were no obvious adverse reactions or complications in the combination group and control group.Conclusion Quyu Huatan Erxian Decoction combined with febuxostat can effectively improve vascular endothelial function and penile arterial blood flow in patients with hyperuricemia, thus erectile dysfunction, without increasing adverse reactions. |
