      目的 研究NLRP3-RIP2-NF-κB信号途径在肺曲菌病患者外周血中的表达水平,及其介导炎性反应的作用机制。方法 收集2016年5月-2019年10月在福建省老年医院住院肺曲菌病患者62例,其中侵袭性肺曲霉菌病组(IPA组)24例和非侵袭性肺曲霉菌病组(NIPA组)38例,并选取同期20名非真菌感染者作为对照组。采用RT-PCR法检测NLRP3、RIP2、NF-κB mRNA表达水平;Western Blot法检测NLRP3、RIP2、NF-κB的蛋白水平;采用ELISA检测各组的炎症因子hs-CRP、IL-6水平。结果 1.RT-PCR结果显示IPA组和NIPA组中NLRP3、RIP2、NF-κB mRNA的表达水平均较对照组升高,且IPA组升高最明显,具有显著性差异 (P<0.01)。2. IPA组和NIPA组中NLRP3、RIP2、NF-κB 蛋白的表达水平均较对照组升高,且IPA组升高最明显,具有显著性差异(P<0.01)。3. ELISA结果显示IPA组和NIPA组炎症指标IL-6、hs-CRP与对照组比较均升高,差异具有统计学意义(P<0.05);IPA组较其他两组明显升高,具有显著性差异(P<0.01)。4. 相关性分析发现外周血中NLRP3与下游基因RIP2、NF-κB mRNA呈正相关(相关指数r=0.676,P<0.001;r=0.513,P<0.001);与炎症因子IL-6、hs-CRP与NLRP3之间呈正相关(相关指数r=0.467,P<0.001;r=0.621,P<0.001)。结论 肺曲菌病患者外周血中NLRP3、RIP2、NF-κB mRNA和蛋白表达均升高,NLRP3可能通过RIP2依赖的NF-κB信号通道介导肺曲菌病患者致炎过程。

      Objective To study the expression level of NLRP3-RIP2-NF-κB signaling pathway in peripheral blood of patients with pulmonary aspergillosis and its mechanism of mediating inflammatory response. Methods From May 2016 to October 2019, 62 patients with pulmonary aspergillosis were hospitalized in the elderly hospital of Fujian Province, including 24 in the invasive pulmonary aspergillosis group (IPA group) and the non-invasive pulmonary aspergillosis group (NIPA Group) 38 cases, and 20 non-fungal infections were selected as the control group. RT-PCR was used to detect NLRP3, RIP2, NF-κB mRNA expression levels; Western Blot method was used to detect NLRP3, RIP2, NF-κB protein levels; ELISA was used to detect the levels of inflammatory factors hs-CRP, IL-6 in each group. Results 1. RT-PCR results showed that the expression levels of NLRP3, RIP2, NF-κB mRNA in the IPA group and the NIPA group were higher than those in the control group, and the IPA group had the most significant increase, with a significant difference (P<0.01). 2. The expression levels of NLRP3, RIP2, NF-κB protein in the IPA group and the NIPA group were higher than those in the control group, and the IPA group had the most significant increase, with a significant difference (P<0.01). 3. The ELISA results showed that IL-6 and hs-CRP in the IPA group and the NIPA group were significantly higher than those in the control group, and the difference was statistically significant (P<0.05). The IPA group was significantly higher than the other two groups and had a significant difference. Sexual difference (P<0.01). 4. Correlation analysis found that NLRP3 in peripheral blood was positively correlated with downstream genes RIP2 and NF-κB mRNA (correlation index r=0.676, P <0.001; r=0.513, P<0.001); and inflammatory factors IL-6, hs-CRP and There was a positive correlation between NLRP3 (correlation index r=0.467, P<0.001; r=0.621, P<0.001).. Conclusion The expression of NLRP3, RIP2, NF-κB mRNA and protein in peripheral blood of patients with pulmonary aspergillosis are increased. NLRP3 may mediate the inflammatory process of patients with pulmonary aspergillosis through the RIP2-dependent NF-κB signal channel.
