| 目的 探究栀子醇提取物联合头孢哌酮舒巴坦(SCF)对产超广谱β内酰胺酶肺炎克雷伯菌(ESBLs-KP)的体外抑菌效果,为临床治疗 ESBLs-KP 感染提供新的思路和方法。方法 收集2024年6月至2025年6月我院24株非重复分离的ESBLs-KP菌株。实验分为栀子醇提取物组、头孢哌酮舒巴坦组、栀子醇提取物与头孢哌酮舒巴坦联用组。将栀子醇提取物组倍比稀释成连续浓度梯度药液,通过纸片扩散法(K-B 法)测定不同浓度栀子醇提取物的抑菌圈直径,并运用微量肉汤稀释法测定其体外最低抑菌浓度(MIC);头孢哌酮舒巴坦组按同样方法倍比稀释成连续浓度梯度药液,微量肉汤稀释法测定其MIC;两药联用组通过棋盘稀释法设计体外抑菌试验,测定不同浓度组合下的MIC,计算部分抑菌浓度指数(FICI)以判定联合使用效应。 结果 栀子醇提取物的抑菌圈直径呈现出随浓度升高而逐步增大的趋势(P < 0.05)。栀子醇提物浓度介于750~3000 mg/mL对ESBLs-KP表现出敏感。栀子醇提物联合头孢哌酮舒巴坦其MIC均显著降低(P<0.05),其中头孢哌酮舒巴坦 MIC 均值由单用时的(144.00±43.24)μg/mL降至联合时的(20.67±11.04)μg/mL,而栀子醇提取物MIC均值由单用时的(843.75±253.37) mg/mL 降至联合时的(281.25±135.43) mg/mL;栀子醇提物与头孢哌酮舒巴坦联合 FICI 均值为(0.47±0.13)。 结论 栀子醇提取物与头孢哌酮舒巴坦联合应用能显著提高抑制ESBLs-KP生长的效果,其为临床新的联合用药提供了实验依据。 |

| Objective: We aimed to investigate the in vitro antibacterial effect of Gardenia jasminoides ethanol extract in combination with cefoperazone sulbactam against extended-spectrum β-lactamase-producing Klebsiella pneumoniae (ESBLs-KP), offering new insights and methods for the clinical treatment of ESBLs-KP infections.
Methods: From June 2024 to June 2025, 24 non repetitive ESBLs KP strains were collected in our hospital. The experiment was divided into Gardenia alcohol extract group, cefoperazone sulbactam group, gardenia alcohol extract combined with cefoperazone sulbactam group. The inhibition zone diameter of Gardenia alcohol extract at different concentrations was determined by disk diffusion method (K-B method), and the minimum inhibitory concentration (MIC) in vitro was determined by broth microdilution method; The MIC of cefoperazone sulbactam group was determined by broth microdilution method; In the two drug combination group, the in vitro antibacterial test was designed by checkerboard dilution method, the mic under different concentration combinations was measured, and the partial inhibitory concentration index (Fici) was calculated to determine the effect of combined use.
Results: As the concentration increased, the antibacterial zone diameter of the Gardenia jasminoides ethanol extract expanded, demonstrating statistical significance (P < 0.05). Between concentrations of 750 and 3000 mg/mL, the extract showed sensitivity to ESBLs-KP. When combined with cefoperazone sulbactam, the MIC of the extract was notably reduced (P < 0.05). The average MIC of cefoperazone sulbactam decreased from (144.00 ± 43.24) μg/mL to (20.67 ± 11.04) μg/mL, and the MIC of the extract decreased from (843.75 ± 253.37) mg/mL to (281.25 ± 135.43) mg/mL. The mean FICI for the combination was 0.47 ± 0.13.
Conclusions: The combination of Gardenia jasminoides ethanol extract with cefoperazone sulbactam has demonstrated synergistic and additive effects, exhibiting significant in vitro antibacterial activity against ESBLs-KP. This presents a potential new combination therapy. |
