      摘要:目的 观察智能仿生排痰系统对RICU呼吸机依赖患者气道廓清及肺功能指标的疗效,构建科学的气道廓清方案,指导临床实践。方法 采用病例对照研究设计,选取2020年1月至2021年12月南昌大学第一附属医院RICU呼吸机依赖患者60名为研究对象,将2020年1月-2020年12月收治的患者列为对照组,2021年1月-2021年12月收治的患者列为观察组,每组各30例。对照组采用雾化后振动排痰方式,干预组在对照组基础上加用仿生排痰系统,记录干预过程并对结局指标进行统计分析。结果 两组血氧变异值、心率变异值、呼吸变异值的比较差异无统计学意义(P>0.05),两组氧合指数、肺顺应性、肺阻力、痰液量、痰液粘稠度和痰液颜色的比较差异具有统计学意义(P<0.05)。结论 智能仿生排痰系统短期内对于痰液清除、改善机械通气和稳定生命体征方面具有良好效果,且对不同通气阻抗类型患者效果均良好,是呼吸机依赖患者新型有效的气道廓清方式。

      Abstract: Objective To observe the efficacy of the intelligent bionic expectoration system in airway clearance and pulmonary function indicators of patients dependent on RICU ventilators, construct a scientific airway clearance program, and guide clinical practice. Methods: A case-control study design was adopted. Sixty patients dependent on ventilators in the RICU of a tertiary hospital from January 2020 to December 2021 were selected as the research subjects. Patients admitted from January 2020 to December 2020 were listed as the control group, and those admitted from January 2021 to December 2021 were listed as the observation group, with 30 cases in each group. The control group adopted the vibration expectoration method after atomization, while the intervention group added a bionic expectoration system on the basis of the control group. The intervention process was recorded and the outcome indicators were statistically analyzed. Results: There was no statistically significant difference in the comparison of blood oxygen variation value, heart rate variation value and respiratory variation value between the two groups (P>0.05), while there were statistically significant differences in the comparison of oxygenation index, lung compliance, lung resistance, sputum volume, sputum viscosity and sputum color between the two groups (P<0.05). Conclusion: The intelligent bionic expectoration system has a good effect on sputum clearance, improvement of mechanical ventilation and stabilization of vital signs in the short term, and has a good effect on patients with different ventilation impedance types. It is a new and effective airway clearance method for patients dependent on ventilators.
