目的:探究中医经方配合西医对慢性心力衰竭(CHF)的治疗价值。 方法:选取76例于甘肃省庆阳市环县中医医院诊治的辨证属阳虚水泛证的CHF患者,随机对其分组,对照组、试验组病例数均为38例。对照组施以“新四联”方案治疗,试验组在“新四联”基础上,辅以葶苈大枣泻肺汤合真武汤治疗,对治疗后各组中医证候总分,临床疗效,心功能指标,实验室指标予以比较。结果:治疗后组间比较,试验组中医证候总分更低(P<0.05);治疗总有效率试验组(94.74%)较对照组(78.95%)更高(P<0.05);治疗后组间比较,试验组左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVESD)更小,左心室射血分数(LVEF)更高(P均<0.05);治疗后组间比较,试验组血清血管紧张素Ⅱ(AngⅡ)、N-脑钠肽前体(NT-proBNP)、 同型半胱氨酸(Hcy)、高敏心肌肌钙蛋白Ⅰ(hs-cTnⅠ)水平更低(P均<0.05)。结论:葶苈大枣泻肺汤合真武汤用于CHF治疗中,可提升疗效,利于症状恢复,并能促进心功能及实验室指标改善。 |

Objective: To explore the therapeutic value of traditional Chinese medicine (TCM) combined with Western medicine in the treatment of chronic heart failure (CHF). Methods: A total of 76 CHF patients with Yang deficiency and water pan syndrome who were diagnosed and treated in Huanxian Hospital of Traditional Chinese Medicine, Qingyang City, Gansu Province were randomly divided into groups, and the number of cases in the control group and the experimental group was 38. The control group was treated with the "New Quadruple" regimen, and the experimental group was treated with the "New Quadruple" regimen, supplemented by the total TCM syndrome score, clinical efficacy, cardiac function indexes and laboratory indicators of each group after treatment. Results: Compared with the experimental group after treatment, the total score of TCM syndrome in the experimental group was lower (P<0.05), the total effective rate of treatment in the experimental group (94.74%) was higher than that in the control group (78.95%) (P<0.05), and the left ventricular end-diastolic diameter (LVEDD) and left ventricular end-systolic diameter (LVESD) were smaller in the experimental group after treatment. The left ventricular ejection fraction (LVEF) was higher (all P <0.05), and the serum levels of angiotensin II (AngII.), N-proBNP, homocysteine (Hcy) and high-sensitivity cardiac troponin I (hs-cTnI.) were lower in the experimental group (all P <0.05). Conclusion: The use of Zhenwu decoction in the treatment of CHF can improve the efficacy, facilitate the recovery of symptoms, and promote the improvement of cardiac function and laboratory indicators. |
