| 目的 观察桃红四物汤合五苓散内服结合冰硝散外敷在膝关节周围骨折术后早期应用的临床疗效及对血清中炎症因子水平的影响。方法 将我院收治的60例膝关节周围骨折手术患者随机分为治疗组和对照组，每组均为30例。2组术后给予常规治疗及康复锻炼，疗程2周；治疗组术后予口服桃红四物汤合五苓散为主方的中药汤剂，同时予冰硝散外敷患膝，疗程2周。术后2周比较2组VAS评分、小腿围、膝关节活动度及血清中IL-6、TNF-α的质量浓度，并行Judet膝关节屈曲度和功能评分比较。结果 术后2周，2组VAS评分、小腿围及血清中IL-6、TNF-α的质量浓度比较，治疗组低于对照组，治疗组膝关节活动度高于对照组，差异有统计学意义（P＜0.05）；Judet膝关节屈曲度和功能评分比较，治疗组优良率（86.7%）高于对照组（70%），差异有统计学意义（P＜0.05）。结论 桃红四物汤合五苓散内服结合冰硝散外敷可有效减轻膝关节周围骨折患者术后早期的炎症反应，减轻术后肿胀、疼痛，改善患膝屈曲功能，有效促进患者术后早期康复。 |

| Objective To observe the clinical efficacy of Taohong Siwu Decoction Combined with Wuling powder for oral use and Bingxiao powder for external application in the early postoperative treatment of knee fracture and its influence on the level of inflammatory factors in serum. Sixty patients with peripheral knee fractures were allocated into a treatment group and a control group at random, with 30 individuals in each group. 2 groups were given routine treatment and rehabilitation exercise for 2 weeks after operation. The treatment group was given oral Taohong Siwu Decoction combined with Wuling SAN as the main prescription, and Bingnitosan was applied to the affected knee, the course of treatment was 2 weeks. VAS score, calf circumference, knee range of motion and serum IL-6, TNF-α mass concentration were compared 2 weeks after surgery, and Judet knee flexion and function scores were compared. Outcomes 2 weeks after surgery，the treatment group exhibited lower VAS scores, reduced calf circumference, and decreased serum concentrations of IL-6 and TNF-α when contrasted with the control group. Additionally, the treatment group demonstrated a greater range of knee motion, which was statistically significant when compared to the control group (P<0.05). Compared with Judet knee flexion and function scores, the excellent and good rate of the treatment group (86.7%) was higher than that of the control group (70%), and the difference was statistically significant (P＜0.05). Conclusion Taohong Siwu Decoction Combined with Wuling powder for oral use and Bingxiao powder for external application can effectively reduce the early postoperative inflammatory reaction, reduce postoperative swelling and pain, improve the knee flexion function, and effectively promote the early postoperative rehabilitation of patients with knee fracture. |
