| 目的 建立南昌地区血小板供者库,通过收集已知HPA和HLA基因型的供者信息进行数字配型,为输注无效患者提供匹配血小板,并分析现有库容水平,根据需要进行调整和改进。方法 研究南昌地区无偿献血者的HPA(1-6,10,15,21)和HLA-A/B基因分型情况,采用多重荧光PCR方法对361名献血者进行基因分型,并计算基因频率和单体型频率。推导得到找到≥1例HPA/HLA基因型匹配供者的回归方程。结果 南昌地区361份血小板供者资料库中,经统计分析,约有92%的患者能够找到合适的HPA基因型全匹配供者,大约26%的患者可以找到≥1例相同HLA-A/B供者。此外,通过回归方程分析,得出库容水平与献血者数量、基因型匹配程度等因素有关,需要进一步扩大献血者招募和筛选范围,以降低建库成本并扩充库容。结论 由于不同地区人群的HPA/HLA分布存在差异,需要根据实际情况和需求制定合适的血小板供者库的建设方案和最佳库容。有必要扩大供者选择范围,建立南昌地区血小板供者库,以满足更多患者的需求。这一研究结果对于输血医学和遗传学领域都具有重要的意义。 |

| Objective To establish the database of platelet donors with known HPA and HLA genotypes in Nanchang, and analyze the storage level, so as to provide digital matching for patients with platelet transfusion refractoriness. Methods The genotypes of HPA(1-6,10,15,21) and HLA-A, -B of 361 voluntary blood donors in Nanchang were detected by multiplex fluorescent PCR, and the gene frequencies and haplotype frequencies were calculated. A regression equation was derived to find at least one HPA/HLA matched donor. Results The database of platelet donors in Nanchang area was established. Among 361 donors with known HPA(1-6,10,15,21)genotypes, about 92% of the patients could be found HPA genotype matched donors. Among 361 platelet donors with known HLA-A, -B genotypes, the probability of finding at least one identical donor was about 26%. Conclusion |
