      目的:本研究将超声引导液压扩张法与可视化针刀联合应用于对冻结肩的治疗中,再结合手法松解治疗,并集中对该治疗方案的应用效果进行分析。方法:研究对象为我院骨伤科在2022年1月至2024年1月期间收治的84例符合排纳标准的冻结肩患者,随机等分为实验组(n=42)和参比组(n=42)。两组患者均接受一般治疗,参比组实施超声引导液压扩张法及手法松解治疗,实验组在参比组的基础上实施可视化针刀治疗。每2周治疗1次、4周为1疗程。治疗结束后对比两组的疼痛程度评分(VAS)、肩关节功能评分(CMS)、睡眠质量指数(PSQI)以及临床疗效。结果:治疗前,两组VAS、CMS、PSQI评分对比无差异(P>0.05),治疗后1、2、4周,两组VAS、PSQI评分较治疗前均有改善,呈阶梯式下降,其中实验组均低于参比组;而CMS评分呈阶梯式升高,其中实验组均高于参比组;治疗后实验组临床有效率为95.24%,高于参比组76.19%,(P<0.05)。结论:在对冻结肩进行治疗时可采取超声引导液压扩张法联合可视化针刀及手法松解治疗的方案。该方案能够有效减轻患者的疼痛感、提高患者的肩关节功能,改善患者的睡眠质量,提高临床疗效。

      Objective: In this study, ultrasound-guided hydraulic expansion combined with visualized acupotomology was applied in the treatment of frozen shoulder, combined with manual release, and the application effect of the treatment plan was analyzed. Methods: A total of 84 patients with frozen shoulder who met the exclusion criteria and were admitted to the Department of Orthopedics and Trauma of our hospital from January 2022 to January 2024 were randomly divided into an experimental group (n=42) and a reference group (n=42). Both groups received general treatment, the reference group was treated with ultrasound-guided hydraulic expansion and manual release, and the experimental group was treated with visualized acupotomology on the basis of the reference group. The treatment was given once every 2 weeks and 4 weeks was a course of treatment. After treatment, the pain degree score (VAS), shoulder function score (CMS), sleep quality index (PSQI) and clinical efficacy were compared between the two groups. Results: Before treatment, there was no significant difference in VAS, CMS, and PSQI scores between the two groups (P > 0.05). At 1, 2, and 4 weeks after treatment, the VAS and PSQI scores of the two groups were improved, showing a stepwise decline, and the scores in the experimental group were lower than those in the reference group. The CMS scores increased in a stepwise manner, and the CMS scores of the experimental group were higher than those of the reference group. After treatment, the clinical effective rate of the experimental group was 95.24%, which was higher than 76.19% of the reference group (P < 0.05). Conclusion: Ultrasound-guided hydraulic expansion combined with visualized acupotomology and manual release can be used in the treatment of frozen shoulder. The program can effectively reduce the pain of patients, improve the shoulder function of patients, improve the sleep quality of patients, and improve the clinical efficacy.
