      目的:观察显微手术结合早期功能锻炼治疗肌腱断裂临床效果及对手功能影响。方法:研究纳入显微外科收治的102例手指肌腱断裂患者分组研究,以随机数字表法分为观察组和对照组,各51例。对照组采用常规手术方案进行治疗,并对患者进行早期功能锻炼;观察组采取显微手术治疗,并术后进行早期康复锻炼。比较治疗后两组治疗效果,术后8 h、术后24 h与术后48 h患者视觉模拟评分法(VAS)评分,术后并发症发生情况,治疗前及治疗4周后手功能量表评分变化,治疗4周后手指外观与血液循环状态评分情况,治疗前及治疗4周后患者生活质量评分变化。结果:治疗前两组手功能量表评分、生活质量评分比较,无显著差异(P>0.05);治疗4周后两组手功能量表评分、生活质量评分均改善,且观察组优于对照组(P<0.05);观察组治疗总优良率(88.24%)高于对照组(72.55%);术后8 h、术后24 h与术后48 h的VAS评分均低于对照组,术后并发症发生率低于对照组,手指外观与血液循环状态评分优于对照组(P<0.05)。结论:显微手术结合早期功能锻炼治疗肌腱断裂临床效果显著,患者并发症较少,术后疼痛轻,患者手功能与手外观、生活质量均改善。

      Objective: To observe the effect of microsurgery combined with early functional exercise on tendon rupture and hand function. Methods: The study included 102 patients with finger tendon rupture admitted to microsurgery and divided them into observation group and control group by random number table method, with 51 cases in each group. Patients in the control group were treated with conventional surgery and received early functional exercise; patients in the observation group were treated with microsurgery and received early postoperative rehabilitation exercise. To compare the therapeutic effect of the two groups after treatment. Patients' pain score at 8 h, 24 h and 48 h after visual analog scale (VAS), postoperative complications, changes of hand function scale score before treatment and 4 weeks after treatment, finger appearance and blood circulation status score after 4 weeks after treatment, changes of quality of life scale score before treatment and 4 weeks after treatment. Results: Before treatment, there were no significant differences in hand function scale score and quality of life score between the two groups (P>0.05). After 4 weeks of treatment, hand function scale scores and quality of life scores of both groups were improved, and the observation group was better than the control group (P<0.05). The total excellent and good rate of observation group (88.24%) was higher than that of control group (72.55%). VAS scores were lower than control group at 8 h, 24 h and 48 h after surgery, the incidence of postoperative complications was lower than control group, and the scores of finger appearance and blood circulation status were better than control group (P<0.05). Conclusion: Microsurgery combined with early functional exercise has significant clinical effect in the treatment of tendon rupture, with fewer complications, less postoperative pain, and improved hand function, hand appearance and quality of life.
