      【摘要】目的:探究重复经颅磁刺激(rTMS)与高压氧联合治疗对颅脑损伤康复期患儿神经功能的影响。方法:选用2021年7月-2022年12月于我院儿科收治的颅脑损伤恢复期患儿共82例为研究对象,按随机对照组法分为试验组和对照组各41人,其中对照组采用重复经颅磁刺激(rTMS)治疗,试验组使用重复经颅磁刺激(rTMS)联合高压氧治疗,采用神经功能评分(NIHSS)、Fugl-Meyer运动功能测评法(FMA)评分及认知功能评定表(LOTCA)来评价两组患儿治疗前后神经功能康复的情况。结果:两组患者治疗后NIHSS评分较治疗前均降低,且治疗后试验组评分低于观察组(P<0.05);两组患儿上下肢FMA评分治疗后均高于治疗前,治疗后试验组FMA评分高于对照组(P<0.05);两组患儿LOTCA评分治疗后均比治疗前显著提高且试验组高于对照组(P<0.05)。结论:rTMS联合高压氧治疗颅脑损伤康复期患儿,能有效提高患儿认知及运动能力,促进神经功能恢复。

      Abstract [Objective] To explore the clinical effect of repetitive transcranial magnetic stimulation (rTMS) combined with hyperbaric oxygen on recovery of neurological function in children with craniocerebral injury during convalescence period. Methods Eighty-two children with craniocerebral injury during convalescence period in pediatrics department of our hospital from July 2021 to December 2022 were enrolled, and classified into two groups by random number table methods, each with 41 cases. Control group received rTMS therapy, while experimental group received rTMS therapy combined with hyperbaric oxygen. Then the neurological function of children were assessed using National Institutes of Health Stroke Scale (NIHSS), Fugl-Meyer assessment (FMA) and Loewenstein Occupational Therapy Cognitive Assessment (LOTCA) before and after treatment. Results After treatment, a reduction was detected in NIHSS score in both groups (P<0.05), and experimental group scored lower on NIHSS than control group (P<0.05). FMA scores of upper and lower limbs showed an increase in both groups after treatment (P<0.05), and the increase was more remarkable in experimental group than in control group (P<0.05). An increase in LOTCA score was detected in both groups after treatment (P<0.05), and LOTAC score of experimental group was higher than that of control group (P<0.05). Conclusion Application of rTMS combined with hyperbaric oxygen for children with craniocerebral injury during convalescence period can effectively improve the cognitive and motor abilities, thus promoting the neurological recovery.
