| 目的：研究免疫抑制剂结合生物制剂对类风湿关节炎（RA）病人的作用。方法：挑选2019年8月～2021年10月期间于我院治疗的RA98例病人作为此次试验的对象，随机分为观察组、对照组，两组均采取来氟米特进行治疗，观察组病人在此基础上联合采取阿达木单抗注射液治疗，治疗8周，比较两组病人的治疗效果、免疫相关指标、炎性相关指标、骨代谢相关指标、血清相关标志物、生活质量之间的差异。结果：治疗结束后，两组病人的中医症状方面的评分、晨僵的具体时间、关节压痛数量、肿胀关节数量以及DAS28评分显著改善，且观察组病人的中医症状方面的评分、晨僵的具体时间、关节压痛数量、肿胀关节数量以及DAS28评分显著低于对照组（P＜0.05）；两组病人的补体C3、补体C4、IgA、IgG、IgM均显著升高，且观察组病人的补体C3、补体C4、IgA、IgG、IgM显著高于对照组，两组病人的血清炎性相关指标均显著下降，且观察组病人的血清炎性相关指标显著低于对照组（P＜0.05）；两组病人的ALP、ⅠCTP、PⅠCP、BGP均显著下降，且观察组病人的ALP、ⅠCTP、PⅠCP、BGP显著低于对照组；两组病人的RF、anti-CCP、OPN均下降明显，且观察组病人的RF、anti-CCP、OPN较对照组病人下降明显（P＜0.05）；生活质量方面，观察组病人较对照组病人更高（P＜0.05）。结论 采用免疫抑制剂结合生物制剂对RA病人进行治疗，病人的骨代谢以及血清学指标显著改善，治疗效果显著。 |

| objective: To investigate the effect of immunosuppressive agents combined with biological agents on patients with rheumatoid arthritis. Methods: A total of RA98 patients treated in our hospital from August 2019 to October 2021 were selected as the subjects of this trial and randomly divided into observation group and control group.Both groups were treated with leflunomide, and patients in the observation group were treated with adalimumab injection for 8 weeks. The therapeutic effect, immune related indexes, inflammatory related indexes, bone metabolism related indexes, serum related markers, and quality of life were compared between the two groups. Result：After treatment, the TCM symptom score, morning stiffness time, number of joint tenderness, number of swollen joints and DAS28 score of 2 groups were obvious improved, and the TCM symptom score, time of morning stiffness, number of joint tenderness, number of swollen joints and DAS28 score of observation group were obvious lower than control group (P < 0.05). The complement C3, complement C4, IgG, IgA and IgM of 2 groups were obvious increased, and the complement C3, complement C4, IgG, IgA and IgM of the observation group were obvious higher than control group, and the TNF-α , IL-1β and IL-6 of 2 groups were obvious descended. The levels of TNF-α , IL-1β and IL-6 and PCT in observation group were obvious lower than control group, and the differences were statistically obvious (P < 0.05). The ALP, ⅰ CTP, Pⅰ CP and BGP of 2 groups were obvious descended, and the ALP, ⅰ CTP, Pⅰ CP and BGP of observation group were obvious lower than control group. RF, anti-CCP and OPN of 2 groups were obvious descended, and the RF, anti-CCP and OPN of observation group were obvious lower than control group (P < 0.05). Conclusion: Immunosuppressive agents combined with biological agents were used to treat patients with rheumatoid arthritis. Bone metabolism and serological indexes of patients were obvious improved, and the treatment effect was obvious. |
