      [摘要] 目的:探讨生肌创愈膏对大鼠感染性创面愈合的影响及可能作用机制。方法:选取 32只大鼠,分为正常创面组、模型组、湿润烧伤膏组、生肌创愈膏组,每组 8只。在大鼠背部腰椎偏上处做两个直径1.5cm的全层皮肤缺损创面,除正常创面组,其他大鼠通过创面接种金黄色葡萄球菌液+腹腔注射醋酸氢化可的松的方法制备感染性创面模型。待造模成功后,正常创面组和模型组予以生理盐水纱布换药,湿润烧伤膏组予以湿润烧伤膏纱布换药,生肌创愈膏组予以生肌创愈膏纱布换药,每天换药2次。观察并记录各组大鼠给药第 3 天、第 7 天大鼠一般情况及创面情况; 分别在给药第3天、第7天随机取各组大鼠创面组织,进行苏木精-伊红染色(HE染色)观察大鼠创面组织病理组织学改变,通过酶联免疫法( ELISA) 检测创面组织中肿瘤坏死因子-α( TNF-α) 、白细胞介素-6( IL-6) 含量。结果:给药第3天、第7天时,湿润烧伤膏组和生肌创愈膏组大鼠一般情况和创面生长情况均优于模型组;与湿润烧伤膏组相比,生肌创愈膏组创面渗液较少,上皮爬行更明显,愈合情况更好;给药第 3天、第7天,模型组、湿润烧伤膏组及生肌创愈膏组大鼠创面组织中 TNF-α、IL-6 含量均显著高于正常创面组( P<0.05) ; 给药第3天、第 7 天,与模型组相比,湿润烧伤膏组及生肌创愈膏组大鼠创面组织TNF-α、IL-6含量明显下降 ( P<0. 05) ;给药第7天时,生肌创愈膏组TNF-α、IL-6含量明显低于湿润烧伤膏组 ( P<0.05) 。结论:生肌创愈膏可加快组织修复,促进大鼠感染性创面愈合,其机制可能与降低创面 TNF-α、IL-6 表达水平,抑制创面炎症反应有关。

      [Abstract] Objective: To explore the healing effect of Shengji Chuangyu ointment on infected wounds in rats and its possible mechanism. Method: Thirty-two rats were selected and divided into normal wound group, model group, MEBO group and Shengji Chuangyu ointment group. Two traumas of 1.5 cm in diameter with full skin defects were made on the rat"s dorsal back just above the lumbar vertebrae. Except for the normal wound group, infected wound models were prepared by by noculation of Staphylococcus aureus and intraperitoneal injection of hydrocortisone acetate in rats. After successful modeling, the normal wound group and the model group were given saline yarn,, the MEBO group were given MEBO yarn, and the Shengji Chuangyu ointment group were given Shengji Chuangyu ointment yarn, twice a day. The general condition of the rats and the traumatic condition of the rats on the 3rd and 7th day of treatment were observed and recorded; The wound tissues of each group of rats were randomly taken on the 3rd and 7th day of treatment, and hematoxylin-eosin staining was used to detect the pathological histological changes of the wound tissues of rats. The levels of tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) in wound tissues were measured by enzyme-linked immunoassay.Results: On the 3rd and 7th day of treatment, the general condition and wound growth of the rats in the MEBO group and the Shengji Chuangyu ointment group were better than those in the model group;Compared with the MEBO group, the Shengji Chuangyu ointment group had less wound exudate, more pronounced epithelial crawling and better healing. On the 3rd and 7th day of treatment, the TNF-α and IL-6 contents in the wounds of the model group, the MEBO group and the Shengji Chuangyu ointment group were significantly higher than those in the normal wound group (P<0.05); Compared with the model group, the TNF-α and IL-6 contents in the wounds of the MEBO group and the Shengji Chuangyu ointment group were significantly lower (P<0.05); On the 7th day of treatment, the TNF-α and IL-6 contents in the Shengji Chuangyu ointment group decreased significantly than those in the MEBO group (P<0.05). Conclusion : The Shengji Chuangyu ointment can accelerate tissue repair and promote infected wound healing in rats. The mechanism may be related to the reduction of traumatic TNF-α and IL-6 expression levels and inhibition of traumatic inflammatory response.
