目的 探索寻找使用TCI注射泵实施3分钟靶控全麻诱导的依托咪酯血浆浓度(Cp)与瑞芬太尼血浆浓度(Cp)配伍大组中抑制插管反应效果最好又无或少副作用的最佳药物浓度配伍剂量组合。 方法 选择接受气管插管全麻实施骨科、普外科、妇科择期手术ASAⅠ级或Ⅱ级的中青年患者150例,随机分为:依托咪酯Cp1.0ug/ml、Cp1.1ug/ml、Cp1.2ug/ml、Cp1.4ug/ml、Cp1.5ug/ml分别与瑞芬太尼Cp20ng/ml配伍,共5组,每组30例。先静注阿托品0.007 mg/kg ,心率上升后启动TCI泵并开始计时,意识消失时静注罗库溴铵0.8mg/kg,在TCI泵注3分钟时使用Airtraq和Insighters可视喉镜气管插管。记录患者基础和插管前后MAP和HR等数据。 结果 5组中,瑞芬太尼Cp20ng/ml配伍依托咪酯Cp 1.0ug/ml组抑制插管反应效果Ⅱ级,肌颤、肌僵发生率为零。结论 瑞芬太尼Cp20ng/ml配伍依托咪酯Cp 1.0ug/ml组抑制插管反应效果Ⅱ级,肌颤、肌僵发生率为零,是瑞芬太尼Cp配伍依托咪Cp大组中,中青年患者使用TCI注射泵实施3分钟靶控全麻诱导抑制插管反应效果最好(Ⅱ级),又安全的最佳药物浓度配伍剂量组合。 |

Objective To explore and find the optimal combination dose with the best effect of inhibiting intubation response and no or few side effects that use of SLGO CP-800 TCI target pump and general anesthesia induction 3 minutes after to perform endotracheal intubation in etomidate plasma concentration (Cp) and remifentanil plasma concentration (Cp) in the large group. Methods A total of 150 young and middle-aged patients who were selected to receive general anesthesia for tracheal intubation for orthopedic, general surgery, and gynecological elective surgery. It is classified as ASA grade Ⅰor Ⅱ. They were randomly divided into: etomidate Cp1.0ug/ml group,Cp1.1ug/ml group,Cp1.2ug/ml group,Cp1.4ug/ml group and Cp1.5ug/ml group. They were respectively combined with remifentanil Cp20ng/ml.There were 5 groups. Each group was 30 cases. Before the start of TCI pump injection, intravenously inject atropine 0.007mg/kg. After the heart rate rises, start the TCI pump and start timing. When consciousness disappears, intravenously inject rocuronium 0.8mg/kg. When TCI pump injection was used for 3 minutes, the “Airtraq” or “Insighters” visual laryngoscope for tracheal intubation was used. The basic MAP and HR, the MAP and HR before and after intubation were recorded. Results In the 6 groups, remifentanil Cp20ng/ml and etomidate Cp1.1ug/ml compatibility, the effect of inhibiting intubation reaction was grade II, and there were no cases of induced muscle fibrillation and muscle stiffness. Conclusion Young and middle-aged patients were induced by TCI target-controlled pump for 3 minutes. In a large group of remifentanil Cp combined with etomidate Cp, remifentanil Cp 20ng / ml combined with etomidate Cp 1.1ug/ml, which is the best (grade II) and safe combination of drug concentration and dose to inhibit intubation reaction. |
