      目的:探讨黄连与肉桂配伍干预奥氮平介导精神分裂症患者体重增加等代谢障碍的中枢5-羟色胺(5-HT)作用机制。方法:单一服用奥氮平引起体重增加大于10%的首发精神分裂症患者30例,按门诊号顺序随机分配到黄连与肉桂颗粒配伍+奥氮平(A组,15例)组、模拟黄连与肉桂颗粒配伍+奥氮平(B组,15例)组,治疗12周。治疗基线和治疗12周末采用高效液相色谱-电化学检测法测定精神分裂症患者脑脊液5-HT和5-羟吲哚乙酸(5-HIAA)含量;治疗基线和治疗第4、8、12周末测定两组患者体重以及甘油三酯(triacylglycerol, TG)、总胆固醇(total cholesterol, TC)、高密度脂蛋白胆固醇(high density lipoprotein cholesterin, HDL-C)、低密度脂蛋白胆固醇(low density lipoprotein choles cholesterin,LDL-C)水平。结果:治疗基线,两组患者脑脊液5-HIAA含量和5-HIAA/5-HT比值以及体重、TG、TC、HDL-C、LDL-C水平差异无统计学意义(F=0.00, P=0.00); 治疗12周末,A组患者脑脊液5-HIAA含量和5-HIAA/5-HT比值(0.00±0.00)显著高于B组(P<0.01);治疗第12周末,A组患者体重以及TG、TC、HDL-C、LDL-C水平显著低于B组(F=0.00, P=0.00)。结论:黄连与肉桂配伍可能通过上调中枢5-HT神经元活性激活5-HT系统信号通路,抑制摄食量和体重增加。

      Abstract: To explore the effect of the compatibility of Coptis and Cinnamon on the content of 5-HIAA and 5-HIAA/5-HT ratio in the cerebrospinal fluid and the weight gain of schizophrenia patients olanzapine-induced. Method: 30 patients with first-episode schizophrenia who caused a weight gain of more than 10% after taking olanzapine alone. Then, all patients were randomly and evenly assigned to two groups (group A and group B) according to the order of the outpatient number, group A were treated with the combination of Coptis Chinensis and Cinnamon + Olanzapine for 12weeks, group B were treated with Placebo + Olanzapine for 12weeks. Next, we detected the contents of 5-HT and 5-HIAA in cerebrospinal fluid of the patients by high performance liquid chromatography-electrochemical detection method begin and 12 weeks after treatment. Finally, the body weight and the levels of triacylglycerol (TG), total cholesterol (TC), high-density lipoprotein Cholesterol (HDL-C) and low-density lipoprotein cholesterol (LDL-C) of the two groups were measured at the beginning of treatment and the 4th, 8th, and 12th weekends of treatment. Results: At the beginning of treatment, there was no statistically significant difference in body weight and content of 5-HIAA and 5-HIAA/5-HT ratio, TG, TC, HDL-C, and LDL-C levels between the two groups of patients (F=0.00, P=0.00). After 12 Weeks after treatment, content of 5-HIAA and 5-HIAA/5-HT ratio (0.00±0.00) of group A were significantly higher than those of group B (P<0.01), while the body weight of group A as well as levels of TG, TC, HDL- C and LDL-C were significantly lower than group B (F=0.00, P=0.00), Conclusions: our results demonstrate that the compatibility of Coptis and Cinnamon may activate 5-HT system signaling pathways by up-regulating the activity of 5-HT neurons in the central nervous system ,and inhibiting food intake and weight gain.
