      目的:研究妊娠期糖尿病患者血糖控制不达标状态下和体外高糖环境下对Klotho基因表达的影响。方法:选择妊娠期糖尿病患者103例作为研究对象,其中血糖控制不达标56例,血糖控制达标47例;采用酶联免疫吸附剂测定法检测两组血清Klotho蛋白水平并分析妊娠结局,同时在体外高糖环境下培养HEK293细胞,采用逆转录-聚合酶链反应法检测细胞Klotho基因表达水平。结果:血清?琢-Klotho和?茁-Klotho蛋白在妊娠期糖尿病血糖控制不达标组表达水平明显低于达标组(P<0.05);不达标组剖宫产率、巨大儿发生率高于达标组(P<0.05)。逆转录-聚合酶链反应结果表明,高糖可诱导HEK293细胞?琢-Klotho和?茁-Klotho信使核糖核酸表达水平下降。结论:体内外高糖是导致?琢-Klotho和?茁-Klotho表达水平下降的重要因素之一,妊娠期糖尿病的治疗目标为血糖达标。

      Objective: To investigate the effect of Klotho gene expression when blood glucose control is not up to standard in gestational diabetes mellitus patients and high glucose environment in vitro. Methods: 103 patients with gestational diabetes mellitus were selected as the research objects, including 56 cases whose blood glucose control were not up to standard and 47 cases whose blood glucose control were up to standard. Used Enzyme-linked immuno sorbent assay to detect the serum Klotho protein level and analyze the pregnancy outcome. At the same time, HEK293 cells were cultured in high glucose environment in vitro, and the expression level of Klotho gene incells were detected by reverse transcription polymerase chain reaction method. Results: The expression levels of serum ?琢-Klotho and ?茁-Klotho protein in substandard blood glucose group were significantly lower than those in compliance group (P<0.05); cesarean section rate and macrosomia incidence rate in substandard group were higher than those in compliance group (P<0.05). Reverse transcription polymerase chain reaction results showed that high glucose could induce the decrease of the expression of ?琢-Klotho mRNA and ?茁-Klotho mRNA in HEK293 cells. Conclusions: High glucose in vivo or in vitro is one of the important factors leading to the decrease of ?琢-Klotho and ?茁-Klotho expression levels. The most beneficial treatment target of gestational diabetes mellitus is to make the blood glucose up to target.
