      [摘要]目的 探讨环介导等温扩增技术、痰涂片镜检法及固体培养法诊断肺结核病的临床价值。方法 选择2019年4月至2020年4月期间于本院就诊的182例初诊肺结核可疑症状患者,所有入选者均提供3份痰标本,分别进行痰涂片镜检法、固体培养法及环介导等温扩增技术检测。分析环介导等温扩增技术、痰涂片镜检法及固体培养法在肺结核病中的诊断效能,计算灵敏度、特异度、准确度、阳性预测值及阴性预测值。结果 环介导等温扩增技术灵敏度(82.08%)、特异度(86.84%)、准确度(84.07%)、阳性预测值(89.69%)、阴性预测值(77.65%)均高于痰涂片镜检法(46.23%、55.26%、50.00%、59.04%、42.42%),环介导等温扩增技术灵敏度、准确度、阴性预测值均高于固体培养法(58.49%、70.31%、60.00%),差异有统计学意义(P<0.05)。结论肺结核病诊断中环介导等温扩增技术应用价值较高,利于临床提高肺结核病的诊断准确率,在结核病防治机构中具有较好的应用前景。

      [Abstract] Objective To explore the clinical value of loop-mediated isothermal amplification technique, sputum smear microscopy and solid culture method for diagnosis of pulmonary tuberculosis. Methods From April 2019 to April 2020, 182 newly diagnosed patients with suspected symptoms of pulmonary tuberculosis who were treated in our hospital were selected. All enrolled patients were provided with 3 sputum specimens for sputum smear microscopy, solid culture and loop-mediated isothermal amplification technique. The diagnostic efficiency of loop-mediated isothermal amplification technique, sputum smear microscopy and solid culture method in tuberculosis was calculated, and the sensitivity, specificity, accuracy, positive predictive value and negative predictive value were calculated. Results The sensitivity (82.08%), specificity (86.84%), accuracy (84.07%), positive predictive value (89.69%) and negative predictive value (77.65%) of loop-mediated isothermal amplification technique were higher than those of sputum smear microscopy (46.23%, 55.26%, 50.00%, 59.04%, 42.42%). The sensitivity, accuracy and negative predictive value of loop mediated isothermal amplification technique were also higher than those of solid culture (58.49%, 70.31%, 60.00%), and the difference was statistically significant (P<0.05). Conclusion The application of loop-mediated isothermal amplification technology in the diagnosis of pulmonary tuberculosis is of high value, which is beneficial to clinically improve the diagnostic accuracy of pulmonary tuberculosis, and has a good application prospect in tuberculosis prevention and treatment institutions.
