      目的:探讨丹参酮IIA对对2型糖尿病患者合并动脉粥样硬化患者炎症反应的影响。方法:收集2型糖尿病合并AS患者90例,根据随机数字表法分为对照组(45例)和观察组(45例),对照组和观察组均给与常规降血糖治疗(药物治疗),观察组在标准常规治疗的基础上加丹参酮IIA注射液治疗。Luminex200检测两组患者治疗前后血清TNF-α、IL-6、 IL-1β和ICAM-1浓度。体外分离培养健康人及动脉粥样硬化患者外周血单核细胞,分为空白对照组、氧化低密度脂蛋白(ox-LDL)处理组和丹参酮IIA+ox-LDL处理组,通过流式细胞仪检测各组单核细胞凋亡率;RT-PCR检测各组单核细胞中IL-1β mRNA、IL-6 mRNA、TNF-α mRNA和ICAM-1mRNA表达。结果:与对照组及观察组用药前相比,观察组在使用丹参酮IIA 2周后血清TNF-α、IL-6、IL-1β和ICAM-1浓度明显降低(与对照组用药前比:t=3.236、3.655、2.839、1.133;与观察组用药前比:t=2.966、4.755、3.434、7.779,均P<0. 05)。体外通过ox-LDL及丹参酮IIA培养单核细胞,与空白对照组相比,ox-LDL处理组单核细胞凋亡率增加(健康供者:t=6.663;AS患者:t=7.235,P<0.05);与ox-LDL处理组相比,丹参酮IIA+ox-LDL处理组单核细胞凋亡率降低(健康供者:t=4.213;AS患者:t=4.074,P<0.05);与空白组相比,ox-LDL处理组单核细胞中TNF-α mRNA、IL-6 mRNA、IL-1β mRNA和ICAM-1 mRNA表达升高(健康供者:t=6.110、5.710、7.548、12.87;AS患者:t=3.282、3.593、3.279、5.083,P<0.05);与,ox-LDL处理组相比,丹参酮IIA+ox-LDL处理组单核细胞中TNF-α mRNA、IL-6 mRNA、IL-1β mRNA和ICAM-1 mRNA表达降低(健康供者:t=4.681、4.126、6.435、10.42,AS患者:t=2.838、3.312、3.318、4.480,P<0.05)。结论:体内外实验证实参酮IIA可能通过抑制动脉粥样硬化患者疾病过程中的炎症反应,从而减缓动脉粥样硬化进程。

      Objective: To investigate the effect of Tanshinone IIA on inflammatory reaction in patients with type 2 diabetes mellitus complicated with atherosclerosis. Methods: The study enrolled T2DM with AS (90 patients),and divided to control group (45 patients) and observation group (45 patients) according to the method of random number table. Both the control group and the observation group were given routine hypoglycemic therapy (drug treatment), while the observation group was treated with tanshinone IIA on the basis of routine treatment. The levels of serum TNF- α, IL-6, IL-1 β and ICAM-1 in the two groups were measured by Luminex200 before and after treatment. The peripheral blood mononuclear cells from healthy people and atherosclerotic patients were isolated and cultured in vitro. They were divided into blank control group, oxidized low density lipoprotein (ox-LDL) treated group and tanshinone IIA+ox-LDL treated group. The apoptosis rate of monocytes in each group were detected by flow cytometry. The expressions of IL-1β mRNA, IL-6 mRNA, TNF- α mRNA and ICAM-1mRNA in monocytes were measured by RT-PCR. Results: Compared to the control group and the observation group before treatment, the serum levels of TNF- α, IL-6, IL-1 β and ICAM-1 in the observation group were decreased significantly after administration of tanshinone IIA 2 weeks(vs the control group before the treatment ,:t=3.236、3.655、2.839、1.133;vs the observation group before the treatment :t=2.966、4.755、3.434、7.779,P<0. 05). Monocytes were cultured with ox-LDL and tanshinone IIA in vitro. Compared to the blank control group, the apoptosis rate of monocytes in ox-LDL treated group were increased (healthy donor: t= 6.663, AS patient: t=7.235 P<0.05).,while were decreased in tanshinone IIA+ox-LDL treated group(healthy donor::t=4.213;AS patient:t=4.074,P<0.05). The expressions of TNF-α mRNA, IL-6 mRNA, IL-1β and ICAM-1 mRNA in monocytes of ox-LDL treatment were significantly higher than those in control group (healthy donor:t=6.110、5.710、7.548、12.87;AS patient:t=3.282、3.593、3.279、5.083). Compared to ox-LDL treated group, the expression of TNF-α mRNA, IL-6 mRNA, IL-1β mRNA and ICAM-1 mRNA in monocytes of Tanshinone IIA+ox-LDL treatment were decreased(healthy donor:t=4.681、4.126、6.435、10.42,AS patient:t=2.838、3.312、3.318、4.480,P<0.05). Conclusion: The study showed that the tanshinone IIA may slow down the process of atherosclerosis by inhibiting the inflammatory response in vivo and vitro.
