      目的:对比分析3D打印辅助经腹直肌外侧切口入路和传统髂腹股沟入路手术治疗骨盆骨折患者的效果。方法:选择2016年6月至2018年7月间于我院骨科就诊的46例骨盆骨折患者作为本次实验的研究对象,所有患者随机被分为两组,每组23人,对照组采用传统髂腹股沟入路手术,实验组则给予3D打印辅助经腹直肌外侧切口入路手术,对比两组患者治疗前后的髋关节功能及疼痛视觉模拟评分(VAS),对比两组患者手术时间、术后引流量、住院时间和手术切口长度等基本临床指标及术后并发症的发生情况。结果:经过6个月随访,实验组患者髋关节功能优良率显著高于对照组(P<0.05),经不同方法治疗后,实验组髋关节功能评分均显著高于对照组,VAS评分显著低于对照组(P<0.05)。实验组患者手术时间、住院时间均显著短于对照组,术后引流量显著少于对照组,手术切口长度显著短于对照组(P<0.05)。实验组术后并发症总发生率显著低于对照组(P<0.05)。结论:3D打印辅助经腹直肌外侧切口入路相比于传统髂腹股沟入路手术治疗骨盆骨折对于髋关节功能恢复时间短,疗效显著,值得临床广泛使用。

      Objective: To compare and analyze the effect of 3D printing assisted transrectus abdominis lateral incision and traditional ilioinguinal approach in the treatment of pelvic fracture. Methods: 46 patients with pelvic fracture who were treated in orthopaedic department of our hospital from June 2017 to July 2018 were selected as the study subjects. All patients were randomly divided into two groups, the experimental group and the control group according to stratified sampling. 23 patients were in each group. The control group was given traditional ilioinguinal approach surgery, while the experimental group was given 3D printing assisted lateral rectus abdominis incision surgery. The hip joint function and VAS before and after operation were compared. The basic clinical indicators were compared. Results: After 6 months’ follow-up, the excellent rate of hip function in the experimental group was significantly higher than that in the control group (P < 0.05). The score of hip function in the experimental group was significantly higher than that in the control group, and the VAS score was significantly lower than that in the control group (P < 0.05). The operation time and hospitalization time of the experimental group were significantly less than those of the control group, the drainage volume of the experimental group was significantly less than that of the control group, and the length of incision was significantly shorter than that of the control group (P < 0.05). The total incidence of complications in the experimental group was significantly lower than that in the control group (P < 0.05). Conclusion: Compared with the traditional ilioinguinal approach, 3D printing assisted transrectus abdominis lateral incision has less recovery time for hip joint function and better effect. The effect is significant, which is worthy of clinical application.
