      目的 探讨2型糖尿病性骨质疏松患者骨转换标志物的改变,为2型糖尿病患者骨质疏松的诊断和治疗提供参考依据。方法 回顾性对比分析2型糖尿病患者和同期体检健康人群的骨密度( BMD)、空腹血糖( FPG) 、糖化血红蛋白( HbA1C)、血钙及骨转换标志物(BTMs)的差异和关系。 结果 两组检测者随着年龄增加,骨密度均呈下降趋势。2型糖尿病患者股骨颈及Ward’s区骨密度明显低于同龄体检人群,且与年龄及糖尿病病程呈负相关。2型糖尿病患者骨转换标志物与骨密度测定结果有很好的相关性,特别是在股骨颈和Ward’s区骨密度。反映骨形成的标志物如骨钙素(OC)、骨碱性磷酸酶(BALP)、Ⅰ型前胶原氨基末端前肽(PⅠNP) 随骨密度的减低而降低,反映骨吸收的标志物如Ⅰ型胶原蛋白C末端交联肽(CTX)和尿液中吡啶啉(PYD)的含量随骨密度的降低而升高。结论 骨转换标志物在诊断2型糖尿病性骨质疏松上与骨密度测量有很好的拟合性,可早期预示2型糖尿病患者骨质疏松并可用于骨质疏松治疗效果的监测。
英文摘要:
      Objective To investigate the changes of bone turnover markers in type 2 diabetic patients with osteoporosis, and to provide the basis for the diagnosis and treatment of osteoporosis in patients with type 2 diabetes. Method The differences and relationships of bone mineral density (BMD), fasting plasma glucose (FPG), glycated hemoglobin (HbA1C), blood calcium and bone turnover markers (BTMs) were analyzed and compared between healthy population and type 2 diabetes patients using method of retrospective comparative analysis. Result The bone mineral density of the two groups were decreased with age. In type 2 diabetic patients, the bone mineral densities in femoral neck and Ward "s area were significantly lower than that of the health examination group of the same age, and the result was also negatively correlated with age and course of diabetes. Bone turnover markers in type 2 diabetic patients were highly correlated with bone mineral density, especially in femoral neck and Ward "s area. The bone turnover markers which reflected bone formation, such as Osteocalcin (OC), bone alkaline phosphatase (BALP), and procollagen Ⅰ N-terminal peptide (PⅠNP) were all reduced along with the decreasing of bone mineral density. On the contrary, the markers which reflected bone resorption, such as type Ⅰ collagen protein C-terminal crosslinking peptide (CTX) and pyridinoline (PYD) in urine were increased with the decrease of bone density. Conclusion Bone turnover markers were well fitted to the bone mineral density in the diagnosis of type 2 diabetic with osteoporosis. It can early predict osteoporosis in type 2 diabetic patients and can be used to monitor the effect of osteoporosis treatment.
