| 目的 总结分析克氏针髓内固定治疗不同类型腓骨中下段骨折的临床疗效,为腓骨中下段骨折的治疗提供依据。方法 回顾性分析我院自2015年6月-2017年5月采用克氏针髓内固定治疗256例腓骨中下段骨折的临床疗效,其中合并踝关节骨折94例(其中Lauge-Hansen分型旋前外展型9例,旋前外旋型47例,旋后外旋型35例),合并胫骨骨折162例。结果 本研究中256例均获得随访,无一例退钉,美国矫形协会(AOFAS)踝与后足评分系统对患者踝关节功能进行评分(总分100分),末次随访评分中,其中优(90～100分)191例；良(75～89分)60例；可(50～74分)4例；差(50分以下)1例,为44分；总优良率为98.0%；在腓骨中下段骨折克氏针髓内固定的患者中,合并胫骨骨折比合并踝关节骨折的患者术后踝关节功能优良率高。结论 克氏针髓内固定治疗腓骨中下段骨折总体临床疗效确切,合并踝关节不稳定型骨折时,钢板固定可能临床疗效更好。 |

| Objective To summarize and analyze the clinical effect of K-wire intramedullary fixation in the treatment of different types of fracture of the middle and lower fibula, and to provide the basis for the treatment of the fracture of the middle and lower part of the fibula.Methods We retrospectively analyzed the clinical efficacy of 256 cases of mid-to-lower fibula fractures treated with K-wire intramedullary fixation from June 2015 to May 2017 in our hospital. Among them, 94 cases of ankle joint fracture (including 9 cases of Lauge-Hansen type pronation abduction type, 47 cases of pronation and external rotation, 35 cases of retrospin type), and 162 cases of tibial fracture.Results In this study, 256 patients were followed up, and no one had returned. The American Orthopaedic Association (AOFAS) spasticity and posterior-foot scoring system scored the ankle joint function (total score 100 points). In the last follow-up score, excellent (90- 100 points) 191 cases; good (75-89 points) 60 cases; fair(50-74 points) 4 cases; poor (50 points or less) 1 case, 44 points; the overall excellent rate was 98.0%; in the lower humerus Among the patients with intramedullary fixation with fracture Kirschner wire, the rate of good postoperative ankle joint function was higher than that of patients with ankle fracture.Conclusion K-wire Intramedullary fixation for the treatment of the middle and lower Fibula Fractures in the overall clinical effect is correct, combined with unstable ankle fracture, the plate may have better clinical efficacy. |
