| 目的：探讨经后路横行截骨内固定治疗胸腰椎陈旧骨折的临床效果。方法：回顾性分析我院2009年6月~2016年2月采用后路椎体横断截骨结合椎弓根钉内固定治疗的9例胸腰椎陈旧性骨折患者的资料，比较术前、术后患者椎体前缘、后缘高度、Cobb角及VAS评分，评价该方法治疗临床效果及胸腰椎畸形矫正情况。结果：经治疗的患者随访1~3年，平均1年半，椎体前缘、后缘高度分别由术前（18.24±3.21） mm、（20.58±4.93） mm矫正至术后（30.65±3.36） mm、（33.49±3.26） mm；Cobb角由术前（25.95±5.74）°矫正至术后（5.33±3.20）°；VAS评分由术前（6.79±0.65）分矫正至术后（2.04±0.19）分。椎体高度、Cobb角及VAS评分术后与术前相比较差异显著（P＜0.05）。所有患者均骨性愈合，椎体前、后缘高度和Cobb角恢复良好，腰部疼痛症状基本消失，无明显器官损伤等相关手术并发症发生。结论：后路伤椎横形截骨内固定治疗陈旧性胸腰椎骨折有良好的临床治疗效果，脊柱椎体高度及后突畸形矫正令人满意，胸腰椎稳定性无明显破坏，患者运动功能得到保护，恢复快，是一种安全理想的治疗方法。 |

| Objective: To explore the clinical effect of posterior transverse osteotomy and internal fixation for the treatment of the old fracture of thoracolumbar vertebra. Methods: From June 2009 to February 2016, 9 patients were treated with posterior transverse osteotomy and internal fixation, and review the clinical data and therapeutic effect. the mean heights of anterior and posterior borders of the vertebral body、the cobb angles were measured and compared between preoperative and postoperative and the patients-satisfaction was assessed with the visual analogue scale (VAS), the clinical effect and deformity correction were evaluated. Results: The patients were followed for 1 to 3 years (average 18 months). The mean heights of anterior and posterior borders of the vertebral body restored respectively from preoperative (18.24±3.21) mm and (30.58±4.93) mm to postoperative (30.65±3.36) mm、(33.49±3.26) mm, the mean cobb angle was corrected from preoperative (25.95±5.74)° to postoperative (5.33±3.20)°; the VAS score decreased from preoperative (6.79±0.65) to (2.04±0.19). There were significant differences between preoperative and postoperative in the mean heights of the vertebral body、cobb angle、 VAS score (P＜0.05). 9 Cases get well about osseous healing. All the injured vertebrae had get well reposition of height and the Cobb angle. The pain in the waist basically disappeared. No large blood vessels and viscera organ damage cases. Conclusion: For old fracture of thoracolumbar vertebra, the posterior transverse osteotomy and internal fixation operation has good clinical effect. The correction of vertebral height and kyphosis is satisfactory. There is no obvious damage to spinal stability and the movement function is well protected, postoperative recovery is very quick, it is a safe and effective treatment. |
