目的：观察超声刀、单极电刀与冷刀在腹腔镜子宫肌瘤剔除术中的效果差异。方法：回顾性分析2016年1月~2017年12月于我院治疗的92例行腹腔镜子宫肌瘤剔除术患者临床资料，将其中应用超声刀治疗的32例患者设为超声刀组，应用单极电刀的32例患者设为单极电刀组，应用冷刀的28例患者设为冷刀组。比较三组手术时间、术中出血量、子宫缝合时间、术后排气时间、下床活动时间、术后住院时间及术后并发症发生率。结果：与冷刀组相比，单极电刀组和超声刀组手术时间、术中出血量、子宫缝合时间显著缩短（P＜0.05）；单极电刀组和超声刀组手术时间、术中出血量、子宫缝合时间相比较无明显差异（P＞0.05）。结论：腹腔镜子宫肌瘤剔除术中使用超声刀，有助于减少子宫切口出血量，缩短手术时间及子宫缝合时间，促进患者术后恢复，值得临床推广应用。

英文摘要:

Objective: To compare the clinical effects of out ultrasonic scalpel, unipolar electrotome andcold knife in laparoscopic myomectomy for patients with uterine myomas. Methods: In the retrospective analysis of 92 cases of laparoscopic myomectomy patients from January 2016 to December 2017, 28 patients were in cold knife group, 32 patients were in unipolar electrotome group, 32 patients were in ultrasonic scalpel group according to their surgical procedures, and the operation time, intraoperative blood loss, uterus suturetime, ambulation time, postoperative exhaust time, complications, and postoperative hospitalization time were observed and compared. Results: Compared with cold knife group, the operation time, intraoperative blood lossand uterine suture time were significantly decreased in unipolar electrotome and ultrasonic scalpel group(P＜0.05). There is no significant difference for operation time, intraoperative blood loss and uterine suture time between unipolar electrotome group and ultrasonic scalpel group (P＞0.05). Conclusions: The electric equipment like ultrasonic scalpel is helpful in reducing uterine incision blood loss during operation, shortening time of uterus suture and the postoperative recovery in patients with laparoscopic myomectomy. It will be a feasible and safe option to apply the electric equipment in low tiered hospitals, and worthwhile to apply and promote the equipment in these hospitals.
