| 摘要 目的 脓毒症休克患者早期应用血必净，探讨其对血小板活化功能的影响。方法 将我院收治的125例确诊脓毒症休克患者随机分为血必净组（n=65）和常规组（n=60）。血必净组在常规组治疗的基础上给予血必净（100ml，静脉滴注，一天一次），连续给药7天。检测二组患者入院时及入院第7天血小板参数、检测二者血小板膜糖蛋白CD62P、CD63的表达水平。结果 1. 二组患者治疗后血小板计数（PLT）较治疗前升高、血小板平均体积（MPV）、血小板分布宽度（PDW）、大体积血小板的比率（P-LCR）均有所下降；治疗后血必净组各时点数据相较于常规组，改善更明显，二组比较差异有显著性(P<0.01)；2. 治疗后二组患者血小板膜糖蛋白CD62P、CD63的表达水平比较，血必净组各时点数据均低于常规组，差异有非常显著性(P<0.01)。结论 脓毒症休克患者早期即出现严重血小板活化现象；应用血必净治疗可以显著抑制患者血小板的活化。 |

| Objective The effects of early application of Xuebijing on the platelet function of patients with septic shock should be discussed. Methods The 125 patients with septic shock who were admitted to our hospital were randomly divided into Xuebijing group (n = 65) and conventional group (n = 60).Xuebijing group must be treated on the basis of conventional comprehensive treatment (100ml, intravenous infusion, one day) for 7 days. The platelet parameters were detected on the first day of admission and the 7th day of admission. The expression levels of CD62P and CD63 were detected by flow cytometry. Results 1. Two groups after treatment in patients with platelet count (PLT) was increased, the average Xuebijing platelet volume (MPV), platelet distribution width (PDW), mass ratio of platelet (P - the LCR) are decreased; Compared with the conventional group, the data of each time point of the treatment after treatment were significantly improved, and the difference between the two groups was significant (P < 0.01).2. After treatment, the platelet membrane glycoprotein of the two groups was compared, and the data of each time was lower than the conventional group. The difference was very significant (P < 0.01). Conclusions Severe platelet dysfunction was found in patients with septic shock. The application of Xuebijing therapy helps improve platelet function. |
