摘要:目的 探讨慢性阻塞性肺疾病患者血清胰岛素样生长因子I(IGF-I)、生长激素(GH)水平变化并分析IGF-I水平与肺功能、骨密度之间的关系。方法 选择病情稳定的COPD患者70例,健康体检者35例,检测所有受试者血清IGF-I水平、GH水平及肺功能,同时测定COPD患者的骨密度(BMD)。COPD组进一步根据肺功能结果分为轻中度、重度、极重度3组,根据骨密度结果分为骨质疏松、低骨量、正常骨密度组。结果 COPD 患者血清 IGF-I水平明显低于对照组(P<0.01﹚,GH水平与对照组比较差异无统计学意义。肺功能分组比较中,极重度组与健康对照组logIGF-I比较差异具有统计学意义(P<0.01),其余各组间比较无明显差异(P均>0.05),GH水平各组间与对照组两两比较均无显著性差异(P均>0.05)。骨密度分组比较中,各组间logIGF-I差异无统计学意义(P均>0.05)。COPD患者logIGF-I与L1 BMD值( r = 0.298, P =0.015)、L2 BMD值(r = 0.259, P =0.035)、L3 BMD值(r = 0.290, P =0.017)、L4 BMD值(r = 0.289, P =0.018)、左侧股骨干BMD值(r = 0.243, P =0.048)呈直线正相关,与FEV1%预计值、股骨颈BMD值、股骨颈上部BMD值、股骨颈下半部BMD值、Wards三角BMD值、大粗隆BMD值无相关。结论 COPD患者血清IGF-I水平降低,但其水平下降主要发生在极重度患者中,GH水平则无明显变化,IGF-I水平可能与气流受限程度有关;COPD患者血清IGF-I水平与L1、L2、L3、L4、左侧股骨干的BMD值存在相关性。 |

Abstract:0bjective To evaluate the serum levels of insulin-like growth-I(IGF-I) and growth hormone(GH) in patients with chronic obstructive pulmonary disease(COPD) and investigate the relationship between IGF-I and lung function and bone mineral density.Methods 70 stable patients with COPD and 35 healthy controls were admitted,The serum levels of insulin-like growth-I,growth hormone and lung function were measured in all the subjects,in addition,the COPD group also took the bone mineral density measurement.The COPD patients were divided into three groups according to the result of lung function,Then the COPD patients were divided into other three groups(osteoporosis,osteopenia and normal bone mineral density) according to the result of bone mineral density(BMD).Results The levels of IGF-I were found to be significantly lower in COPD group compared to the controls(P<0.01),there was no significant difference in serum GH levels between the two groups.There was significant difference in logIGF-1 between extremely severe COPD patients and healthy controls(P<0.01),there was no significant difference in logIGF-I between the other groups(P>0.05),there was no significant difference too in GH between the groups classified by lung function and healthy controls(P>0.05).There was no significant difference in logIGF-I between the three groups classified by bone mineral density(P>0.05).The bone mineral density of L1(r = 0.298, P =0.015),L2(r = 0.259, P =0.035),L3(r = 0.290, P =0.017),L4(r = 0.289, P =0.018) and left femoral(r = 0.243, P =0.048) were positively correlated with logIGF-I in COPD patients.While logIGF-I in COPD patients was not significantly correlated to BMD of the other measured sites and FEV1% predicted.Conclusion The serum levels of IGF-I were decreased in COPD patients,but the decrease maily occured in extremely severe COPD patients,while the serum levels of GH had barely changed.It means that IGF-I level in COPD patients may change depending on the severity of airflow obstruction. The serum levels of IGF-I were positively correlated with the bone mineral density(BMD) of lumbar(L1-L4) and left femoral. |
