目的：观察纤溶酶联合丁苯酞氯化钠注射液对溶栓时间窗外脑梗死患者血流变及微循环状态的影响和临床疗效。方法：按照随机数字表法将90例脑梗死患者随机分为观察组和对照组，每组45例。在基础治疗的基础上，对照组采用纤溶酶治疗，观察组采用纤溶酶联合丁苯酞氯化钠注射液。治疗前后观测大脑中动脉的峰流速（Vp）、平均流速（Vm）的变化情况以及血流变、微循环状态、NIHSS评分变化和用药安全性指标。结果：同组血流变、微循环状态、Vp、Vm、NIHSS评分治疗前后比较差异有统计学意义（P＜0.05）；观察组血流变、微循环状态、Vp、Vm、NIHSS评分均优于对照组（P＜0.05）。两组患者的用药安全性指标治疗前后均在正常范围内。结论：纤溶酶联合丁苯酞氯化钠注射液可有效改善脑梗死患者血流变、微循环状态、相关血液观察指标和血流动力学，提高急性脑梗死患者的临床疗效，有较高的临床应用价值。

英文摘要:

Objective: To observe the effect of fibrinolytic enzyme combined with Butylphthalide injection on hemorrheology and microcirculation in patients with cerebral infarction after thrombolysis time. Methods: According to the random number table method, 90 patients with cerebral infarction were randomly divided into the observation group and the control group, 45 cases in each group. On the the basic treatment, the control group using fibrinolytic enzyme treatment, the observation group using fibrinolytic enzyme combined with injection. The changes of peak velocity (Vp), mean flow velocity (Vm), blood rheology, microcirculation status and NIHSS score were measured before and after treatment. Result: The results of hemorrheology and microcirculation in the observation group were better than those in the control group, and the difference was statistically significant (P＜0.05). The Vp, Vm and NIHSS scores of the two groups were significantly decreased (P＜0.05). The safety indexes of both groups were in the normal range before and after use. Conclusion: Fibrinolytic enzyme combined with Butylphthalide injection can improve the hemorheology, microcirculation status, hematological parameters and hemodynamics of patients with cerebral infarction, improve the clinical curative effect of patients with acute cerebral infarction, have a higher clinical Value.
