| 【】目的：探讨正骨手法闭合复位结合经皮微创固定(mippo)治疗老年肱骨近端骨折的临床研究。方法：2014年10月至2016年1月,采用正骨手法闭合复位,经皮锁定钢板固定重建肱骨内侧柱稳定治疗老年肱骨近端骨折40例,分为手法组和切复组,记录术后3、6、12个月骨折恢复情况,内固定是否松动断裂,肱骨头有无缺血坏死,内固定取出时间及内固定取出后局部骨质情况、以及有无再骨折。术后应用Constant-Murley评分系统对肩关节进行评定。结果：40例患者均获随访,手法组1例患者术后出现肱骨头内翻畸形,骨折畸形愈合,1例患者出现肱骨头缺血性坏死；切复组2例患者出现肱骨头内翻,1例行肱骨头置换术,另1例行内固定拆除；肩关节功能Constant-Murley评分2组术后比较差异较大,术后12个月根据Constant-Murley评分,两组总分分别为平均为82-93分与77-85分。结论：正骨手法整复经皮微创锁定钢板固定治疗肱骨近端骨折,重建内侧柱稳定,具有创伤小、血循破坏少、固定可靠等优点,是治疗老年性肱骨近端骨折的有效方法。 |

| ABSTRACT Objective：To investtigate the clinical effects of close reduction and percutaneous minimally invasive percutaneous plate osteosynthesis applied in the treatment of proximal humerus in elder patients. Methods：From Jan 2014 to Dec 2016,40 old patients of proximal humerus fractures were treated with locking plate osteosynthesisand rebuild inside of the column stability.They were divided into two groups the Bone-setting manipulation group and Open reduction group．Record 3，6, 12 months after the recovery of the fracture, fracture fixation is loose, the presence or absence of ischemic necrosis of the humeral head, and within a fixed time after removing the fixed local bone removed, as well as whether the re-fracture. The functional outcomes of the shoulder were assessed according to Constant-Murley score．Results：40 patients were followed up.1 patient appeared in the humeral head varus, malunion, 1 patient developed avascular necrosis of the humeral head; the other group humerus head varus observed in 2 patients, 1 underwent humeral head replacement, dismantle the other one internal fixation ; shoulder function Constant-Murley score after March and June relatively large differences, After 12 months, according to the Constant - Murley score two group total score, respectively, for an average of 82-93 and 77-77 points. ; Conclusion：The technique of Laoshi close reduction with percutaneous insertion combined new locking plate internal fixation and rebuilding the inside of the column stability has the advantages of less blood loss,rapid recovery,less vascular damage,stifich infixion and so on．It can effectively treat the proximal humerus fracture of elder patients．
