      目的:评价针刺指标对大脑中动脉梗死(MCAo)大鼠模型的针刺效应的影响,以此来初步探讨针刺治疗脑梗死的作用机制,并为脑梗死针刺治疗方案提供理论依据。方法:本实验以Zea-longa 线拴法复制MCAo大鼠模型。造模成功后依据Zausinger六分法选择1~3分的大鼠随机入组。以随机对照设计原则设立基础对照组(正常组、模型对照组、模型未干预组)和针刺组(内关组、委中组、三阴交组、尺泽组、人中组和非穴组)。而且针刺组内每个穴组采用正交设计法分别设置9个不同参数组合。根据各组别所对应的针刺参数分别进行提插手法针刺治疗,每12小时1次,共干预6次。每组12只,共57组。并以神经行为学、脑血流、梗死率、微循环、光镜等为测量指标,评价针刺指标对MCAo大鼠模型的针刺效应的影响。结果:内关穴在降低梗死率、扩张输入枝管径、提高神经行为学评分表现的最突出;委中穴在增加脑血流、提高神经行为学方面表现最佳;尺泽穴在增加脑血流、扩张输出枝管径、提高正常神经细胞数方面表现最好;三阴交在减少坏死神经细胞和噬神经细胞、降低坏死神经细胞率、提高神经行为学评分方面表现最优。结论:针刺治疗脑梗死疗效确切,其机制可能是通过改善微循环、增加脑血流量、降低梗死率、降低坏死神经元细胞数、减少噬神经细胞数从而提高神经行为学评分来实现的,为脑梗死的针刺治疗方案提供理论依据。

      Objectives: To evaluate the influence of acupuncture indexes on the acupuncture effect in the rat models with the middle cerebral artery occlusion, by which to preliminary explore the mechanism of acupuncture treating cerebral infarction, and provides the theoretical basis for acupuncture treatment of cerebral infarction. Methods: Duplicated MCAO models by Zea-longa's thread ligation assessed by Zausinger's six-point method, and chose rats with 1-3 scores to be grouped. According to the randomized and controlled principle, rats were divided into basic control groups (including normal group, model control group, model group without intervention) and acupuncture groups [including Neiguan (PC6) group, Weizhong (BL40) group, Chize (LU5) group, Sanyinjiao (SP6) group, Renzhong (DU6) group and non-acupoint group]; in each acupoint or non-acupoint 9 different parameter combinations were respectively set by orthogonal intersection method. There were in total 60 groups, each group has 12 rats. And the rats were treated by acupuncture with the lifting-thrusting manipulation every 12 h once, in total 6 times. Indexes of neurobehavioral scores, cerebralblood flow, infarction rate, microcirculation, light microscope, etc. were measured. In order to evaluate the influence of acupuncture indexes on the acupuncture effect in rat models with middle cerebral artery occlusion (MCAo), on the basis of the add-and-subtract standard deviation of means value from all categories in acupuncture group obtained by the application of statistical software SPSS17.0, by the normal group, model control group, model group without intervention as a reference, a comparison was done; after the classified levels were given the weight coefficients the scores were calculated, and then size comparison and sorting were made according to the total scores. Results: The Neiguan (PC6) group is the most prominent performance in lowering infarction rate, expanding input branch pipe diameter and improving neurobehavioral scores. Weizhong (BL40) group is the best in increasing cerebral blood flow and improving neurobehavioral performance. Chize (LU5) group is the best performance in increasing the cerebral blood flow, expanding output branch pipe diameter and increasing the number of normal neural cell. Sanyinjiao (SP6) group is the optimal performance in reducing necrosis of nerve cells and neurophags, reducing the necrosis rate of nerve cells, and improving neurobehavioral scores. Conclusion: the curative effect of acupuncture treatment for cerebral infarction is definite, and its mechanism may be realized through improving microcirculation, increasing cerebral blood flow, decreasing the rate of infarction, reducing the number of necrosis of neurons and decreasing the number of neurophags to improve neurobehavioral scores, and provide theoretical basis for acupuncture treatment for cerebral infarction.
