| 目的：探讨在妇科不孕症手术中运用硬膜外麻结合悬吊式腹腔镜手术的安全性和实用性。方法：将来院就诊的输卵管性不孕症患者138例随机分为两组，观察组在硬膜外麻下行宫腔镜联合悬吊式腹腔镜手术，对照组在全麻下行宫腔镜联合气腹腹腔镜手术，观察术中30 min及术后5 min的平均动脉压（MAP）、心率（HR）、气道峰值压力(PIP)及血氧饱和度（SPO2），观察术后两组患者排气时间和术后疼痛症状。结果：观察组的MAP和PIP明显低于对照组，两组心率和血氧饱和度没有差别。观察组的术后排气时间明显短于对照组，术后疼痛症状的出现人数及术后需吸氧人数明显少于对照组。结论：硬膜外麻下悬吊式腹腔镜手术对病人呼吸和循环系统的影响小于全麻下气腹腹腔镜手术，病人术后症状轻、恢复快，易护理。 |

| Objective:Explore the safety and the application of suspended laparoscopic with epidural anesthesia in gynecologic surgery of infertility.Method:Randomly divided the 138 tubal infertility patients to two groups,the observation group received hysteroscopy combined with suspended laparoscopic under with epidural anesthesia, the control group received hysteroscopy combined with pneumoperitoneum during laparoscopic.MAP,HR, PIP,SPO2 were collected at 30 minutes during operation and 5 minutes after operation.As well,exsufflation time and pain of two groups were observed after operation. Result: MAP and PIP of the observation group was significantly lower than the control group,and there were no differences of HR and SPO2 between two groups. But the exsufflation time of the observation group was shorter than the control group and the number of patients who have pain symptom after operation was less than the control group, as well as the patients who need oxygen after surgery.Conclusion:The effects to respiratory and circulatory system caused by suspended laparoscopic under with epidural anesthesia were smaller than under with pneumoperitoneum during laparoscopic, patients of the observed group were mild symptom, quick recovery, easy care. |
