      目的:探讨重症流行性出血热5期中各阶段的外周血单个核细胞(PBMC)中Fas/FasL表达及相关免疫机制研究。方法 选择经典5期经过的重症出血热患者50例,分为发热期(Ⅰ期组)、低血压休克期(Ⅱ期组)、少尿期(Ⅲ期组)、多尿期(Ⅳ期组)和恢复期(V期组)。AO/EB双染色观察各组PBMC细胞的凋亡率;ELISA法测外周血中TNF-α、IL-6水平;RT-PCR和Western Blot分别测Fas、FasL基因和蛋白的表达。结果:1. AO/EB染色结果:Ⅰ期组、Ⅱ期组、Ⅲ期组PBMC细胞凋亡率明显高于Ⅳ期组和V期组,组间比较均有统计学意义(1h: F=29.12, P<0.001; 6h: F=27.54, P<0.001; 12h: F=25.23, P=0.003; 24h: F=18.17, P=0.005)。2. Ⅰ期组、Ⅱ期组、Ⅲ期组外周血PBMC中的Fas和FasL蛋白表达明显升高,组间比较均有统计学意义(P<0.05);Ⅰ期组、Ⅱ期组及Ⅲ期组PBMC中的Fas和FasL mRNA表达亦明显升高,组间比较均有统计学意义(P<0.05)。3. 前3期的TNF-α、IL-6表达水平明显升高,尤其是I期组炎症因子最高,Ⅳ期组表达量水平相对较低,组间差异有统计学意义(TNF-α: F=42.35, P<0.001; IL-6: F=31.27, P<0.001)。结论:重症流行性出血热的病理过机制存在Fas/FasL介导的细胞凋亡,其浓度高低与机体损害的程度有关。

      Objective: To investigate the expressions of Fas and FasL in peripheral blood mononuclear cells at different stages of severe epidemic hemorrhagic fever and its related immune mechanism. Methods: Fifty patients with severe hemorrhagic fever with classical stage five were enrolled and divided into five groups according to clinical stages: stage Ⅰ group, stage Ⅱ group, stage Ⅲ group, stage Ⅳ group, and stage V group. AO/EB double staining was used to observe the apoptosis rate of PBMC cells in each group. The levels of TNF-α and IL-6 in peripheral blood were measured by ELISA. The expressions of Fas, FasL genes and proteins were measured by RT-PCR and Western Blot, respectively. Results: 1. AO/EB staining results: The apoptotic rate of PBMC cells in stage Ⅰ group, stage Ⅱ group, and stage Ⅲ group was significantly higher than stage Ⅳ and stage V groups, with statistical significance (1h: F=29.12, P<0.001; 6 h: F=27.54, P<0.001; 12 h: F=25.23, P=0.003; 24h: F=18.17, P=0.005). 2. The expression of Fas and FasL proteins in peripheral blood PBMC of stage Ⅰ group, stage Ⅱ group and stage Ⅲ group was significantly increased (P<0.05). The mRNA expressions of Fas and FasL in PBMC of stage Ⅰ, stage Ⅱ and stage Ⅲ groups were also significantly increased (P<0.05). 3. The expression levels of TNF-α and IL-6 in the first three stages were significantly increased, especially the inflammatory factors in the stage I group were the highest, and the expression levels in the stage ⅳ group were relatively low, and the difference was statistically significant (TNF-α: F=42.35, P<0.001; IL-6: F=31.27, P<0.001). Conclusion: Fas/FasL mediated apoptosis exists in the pathogenesis of severe epidemic hemorrhagic fever, and the concentration of Fas/FasL is related to the degree of body damage. It is speculated that apoptosis is the key mechanism involved in the immune damage of Hantavirus.
