| 目的 探讨头侧中间联合入路与中间尾侧联合入路在完整结肠系膜切除术中的应用效果。方法 选取2019年4月-2021年1月于我院行完整结肠系膜切除术的89例结直肠癌患者，按照随机数字表法分为对照组（n=44例）与研究组（n=45例）。对照组采用头侧中间联合入路进行手术，研究组实施中间尾侧联合入路进行手术。比较两组手术时间、术中出血量、住院时间，淋巴结清扫量与并发症。结果 研究组手术时间短于对照组，术中出血量低于对照组（P＜0.05）；两组住院时间无统计学意义（P＞0.05）。两组淋巴结清扫与阳性淋巴结枚数无统计学意义（P＞0.05）。两组肺部感染、肠梗阻、吻合口瘘、淋巴漏等并发症发生率无统计学意义（P＞0.05）。结论 中间尾侧联合入路在完整结肠系膜切除术中安全可行，相对于头侧中间联合入路能显著降低患者术中出血量并缩短手术时间，临床可考虑将该方式作为手术首选方法之一。 |

| To investigate the effect of cephalic middle approach and midcaudal hybrid approach in complete mesocolic excision. Methods Eighty-nine patients with colorectal cancer underwent complete mesocolic excision in our hospital from April 2019 to January 2021 were enrolled, and divided into two groups according to the random number table methods. Control group (n=44) received cephalic middle approach, while study group (n=45) received midcaudal hybrid approach. Then the operation time, intraoperative blood loss, hospital stay length, lymph node dissection and postoperative complications were compared between groups. Results Compared with control group, study group had significantly shorter operation time and smaller amount of intraoperative blood loss (P<0.05). The hospital stay length showed no significant difference between groups (P>0.05). No statistical difference was found in lymph node dissection and the number of positive lymph nodes (P>0.05). The incidence rate of complications including pulmonary infection, intestinal obstruction, anastomotic fistula and lymph leakage showed no significant difference between groups (P>0.05). Conclusion Compared with cephalic middle approach, application of midcaudal hybrid approach in complete mesocolic excision is more safe and effective, which can effectively reduce the intraoperative blood loss and shorten the operation time, so it is worthy of recommendation. |
