      目的:研究NLRP3-RIP2-NF-?资B信号通路在肺曲菌病患者外周血中的表达水平,及其介导炎症反应的作用机制。方法:选取2016年5月~2019年10月收治的肺曲菌病患者62例为研究对象,分为侵袭性肺曲霉菌病组(IPA组)24例和非侵袭性肺曲霉菌病组(NIPA组)38例,并选取同期20例非真菌感染者作为对照组。采用RT-PCR法检测三组NLRP3、RIP2、NF-?资B mRNA表达水平;Western Blot法检测NLRP3、RIP2、NF-?资B蛋白水平;ELISA法检测炎症介质hs-CRP、IL-6水平,对三组上述指标进行比较,并采用Spearman相关性分析分析NLRP3 mRNA与RIP2、NF-?资B、IL-6、hs-CRP之间的相关性。结果:(1)IPA组、NIPA组NLRP3、RIP2、NF-?资B mRNA表达水平均高于对照组,且IPA组高于NIPA组(P<0.05)。(2)IPA组、NIPA组NLRP3、RIP2、NF-?资B蛋白表达水平均高于对照组,且IPA组高于NIPA组(P<0.05)。(3)IPA组、NIPA组IL-6、hs-CRP水平均高于对照组,且IPA组高于NIPA组(P<0.05)。(4)相关性分析发现外周血中NLRP3 mRNA水平与下游基因RIP2、NF-?资B mRNA水平呈正相关(相关指数r=0.676,P<0.001;r=0.513,P<0.001);与炎症介质IL-6、hs-CRP及NLRP3水平呈正相关(相关指数r=0.467,P<0.001;r=0.621,P<0.001)。结论:肺曲菌病患者外周血中NLRP3、RIP2、NF-?资B mRNA和蛋白表达均升高,NLRP3可能通过RIP2依赖的NF-?资B信号通道介导肺曲菌病患者致炎过程。

      Objective: To study the expression level of NLRP3-RIP2-NF-?资B signaling pathway in peripheral blood of patients with pulmonary aspergillosis and its mechanism of mediating inflammatory response. Methods: From May 2016 to October 2019, 62 patients with pulmonary aspergillosis were selected as research objects, they were divided into the invasive pulmonary aspergillosis group (IPA group) with 24 cases and the non-invasive pulmonary aspergillosis group (NIPA Group) with 38 cases, and 20 cases of non fungal infection in the same period were selected as the control group. The expression levels of NLRP3, RIP2, NF-?资B mRNA were detected by RT-PCR. NLRP3, RIP2 and NF-?资B protein levels were detected by Western Blot. The levels of inflammatory mediators hs-CRP, IL-6 were detected by ELISA method. The above indicators of three groups were Compared. The correlation between NLRP3 mRNA, RIP2, NF-?资B, IL-6 and hs-CRP was analyzed by Spearman correlation analysis. Results: (1) The mRNA expression levels of NLRP3, RIP2, NF-?资B in IPA group and NIPA group were higher than those in control group, and IPA group was higher than those in NIPA group (P<0.05). (2) The protein expression levels of NLRP3, RIP2, NF-?资B in the IPA group and the NIPA group were higher than those in the control group, and IPA group was higher than those in NIPA group (P<0.05). (3) The levels of IL-6 and hs-CRP in IPA group and NIPA group were higher than those in the control group, and IPA group was higher than those in NIPA group (P<0.05). (4) correlation analysis showed that NLRP3 mRNA level in peripheral blood was positively correlated with downstream gene RIP2 and NF-?资B mRNA levels (correlation index r=0.676, P<0.001; r=0.513, P<0.001); It was positively correlated with levels of inflammatory mediators IL-6, hs-CRP and NLRP3 (correlation index r=0.467, P<0.001; r=0.621, P<0.001). Conclusion: The levels of NLRP3, RIP2, NF-?资B mRNA and protein expression in peripheral blood of patients with pulmonary aspergillosis were all increased. NLRP3 may mediate the inflammatory process in patients with pulmonary aspergillosis through the RIP2-dependent NF-?资B signaling pathway.
